• Annual Health Report - All adopters must have a veterinarian fill out this form each year that they own the adopted horse.

• Complaint Form - For those who would like to report Abuse & Neglect of a horse, pony, mule, and/or donkey, please print off and use this form when referring a case to Indiana Horse Rescue. Also, please remember to submit photos with dates.

• Donation Agreement - This form is used for owners who are donating a horse to Indiana Horse Rescue. If you are having a horse transported to Indiana Horse Rescue and you will not be accompanying the horse, please make sure that this document is filled out completely and sent with the horse.

• Equine Adoption Agreement - Adopters receive this documentation when adopting an equine from Indiana Horse Rescue. Prior to deciding to adopt an equine from Indiana Horse Rescue, please review our Adoption Agreement so that you are fully aware of our policies for adopted horses.

• Equine Adoption/Foster Application - All individuals interested in Adopting and/or Fostering an equine from Indiana Horse Rescue must have an approved Application on file.

• Equine Donation Questionnaire - We ask that all owners donating any equine to Indiana Horse Rescue fill out our Questionnaire. This is not required, however, by providing us with as much information as possible about the equine, you are allowing us to place that equine in an adoptive home much quicker and more effectively.

• Equine Foster Agreement - All Foster Caregivers receive this documentation upon fostering an equine from Indiana Horse Rescue. Prior to deciding to foster, please review this document so that you are fully aware of the responsibilities of fostering an equine.

• Equine Volunteer Application - We ask that volunteers who are interested in volunteering at Indiana Horse Rescue fill out our Volunteer Application. This form allows us to understand your background and skill level when working with horses. Please do not be alarmed, you do not have to have experience with horses to volunteer, we will teach you what you need to know!

• Fundraising Event Form - Indiana Horse Rescue appreciated and encourages all fundraising efforts. If you are interested in holding a fundraising event for Indiana Horse Rescue, please print off and send in this form. We need to keep tract of all fundraising efforts as accurately as possible, if we know when an event is being held we can list it on our website and inform other about the event!

• Membership Form - Memberships are a great way for people to stay connect to Indiana Horse Rescue, receive the latest news and updates, and help an animal in need, all at the same time. Memberships also make great gifts for the animal lovers in your family!

• Personal Reference Form - All Adoption/Foster Applications must include two Personal References. This form is designed to give your references an idea on what information we are looking for.

• Release of Liability - Any person entering onto an APC facility, especially those with horses must fill out and sign our Release. If your child is visiting the farm with a friend or group, please make sure to print off and sign a Release for them to bring with them or the child will not be allowed to tour the barn.

• Sponsor Form - If you are interested in Sponsoring the care of an equine housed at an Indiana Horse Rescue facility, please print of and send in this form.

• Veterinarian Reference Form - All Adoption/Foster Applications must include on Veterinarian Reference. Please have your current vet or the vet that you intend to use, fill out this form. If you use a university animal hospital such as Purdue University, we understand that it is sometimes very difficult to get a hold of someone to physically fill out the forms, you may submit a copy of your most recent vet bill, if you would prefer.

To provide a caring, positive environment for those horses that have either been neglected, abused, abandoned, and/or are at risk of going to slaughter. We will place these horses in an environment that benefits both the horse and the adoptive family.
