Reference Forms

If you are applying to adopt and/or foster a horse(s) from Indiana Horse Rescue you must submit two personal references and a veterinary reference.

Your references may call in, fax, e-mail, or print out our reference forms. Our forms are here as guidelines for what we would like to know about the care that you give to your current animals. After we have received all of your references we can continue through the approval process.
